After you enroll in a BCBSTX plan, we will send your new plan materials in the mail. You should expect to receive the items below.
Tarjeta de asegurado
You will get a BCBSTX member ID card. Lleve esta tarjeta de asegurado consigo en todo momento.
When you go to your doctor, hospital or other provider, show your BCBSTX member ID card. Tiene la siguiente información:
- Su nombre
- Su número de identificación de asegurado
- Su número del Programa Medicaid
- La fecha de inicio de su cobertura
- El nombre y número de teléfono de su PCP
What if I lose my member ID card?
Call the Customer Advocate Department for your plan to replace your member ID card if it is lost. También puede conectarse a Blue Access for Members℠ to request a new ID card or access your digital ID card.
Kit de bienvenida
After you get your member ID card, you'll get your welcome kit. Este incluye su Manual del asegurado. También recibirá folletos sobre nuestros servicios de transporte y sobre otros servicios. The welcome kit will tell you everything you need to know about being a BCBSTX member.
If you do not receive a welcome kit, we can send a replacement. Please call the Customer Advocate Department for your plan:
- CHIP and STAR Plans, call 1-888-657-6061.
- STAR Kids Plan, call 1-877-688-1811.
We can also provide your kit in formatos alternativos. This can be a language other than English or another format, such as Braille, large print or other electronic formats. We will send you this in your preferred format at no cost to you.
Cuestionario para evaluación de salud
We call new members to do a health assessment over the phone. The assessment will let us know about your health. That way we can send you health tips or tell you about programs we offer to keep you healthy. If you have not received a call to complete a health assessment within 90 days of joining, call the Service Coordination phone number for your plan:
- CHIP and STAR Plans, call 1-877-214-5630.
- STAR Kids Plan, call 1-877-301-4394.
If you are a current member and you have health concerns, you can call Service Coordination. We have health programs for some chronic conditions. You can easily opt-in or opt-out of the programs.
