Forms and Documents Frequently Asked Questions

You can find forms using our convenient Buscador de formularios tool or the Forms & Documents link under the "My Account" tab on Blue Access for Members^SM (BAM).

Download or print the form you need. You can either submit the completed form through the BAM Message Center or mail it to the address listed on the form.

If you're unsure which form to use, you can call customer service at the number on your member ID card to have someone help you choose the right one.

You can also use the Buscador de formularios tool to search for forms by topic. You can scroll through the forms, and click on the title of the form or its icon to see the entire form.

Yes. Texas employers, whether their BCBSTX plan is for a small, mid-market or large group, can find commonly used forms on the employer-specific version of Buscador de formularios. Employers can also find forms on the Formularios y documentos page in the employer section of our website.

Employers with group plans can also find all the tools they need, including forms, on Blue Access for Employers^SM (BAE). This secure portal was designed for benefit administrators of employer groups with BCBSTX health insurance plans. Con BAE, employers can perform online transactions for enrollment, membership, reporting, administration, billing and more — 24/7.

Producers with group plans can also find all the tools they need, including forms, on Blue Access for Producers^SM (BAP). This secure portal was designed for benefit administrators of producer groups with BCBSTX health insurance plans.   Buscador de formularios has the same forms on it as those in BAM.

To file a medical claim for reimbursement, download the Claim Form – Medical. To file a dental claim for reimbursement, download the Formulario de reclamación - Beneficios dentales. To file a prescription claim for reimbursement, dowload the Formulario de reclamación - Medicamentos con receta.   

A standard authorization form lets you decide how BCBSTX can share your protected health information (PHI). You can list the names of all the individuals you'd like to have access to your PHI. You can also set a timeframe for when their access ends and choose what PHI is shared with them.

For example, if a family member is helping with your care, you can add their name to your standard authorization form. Other individuals listed may include your:

• Friend
• Lawyer
• Caregiver

You can change or cancel your standard authorization form at any time. If you don't want your information shared with anyone, there's no need to use a standard authorization form. Visite BAM to print new copies of the form.

You can sign up for automatic bill pay by completing the Auto Bill Pay – Automatic Premium Payment Authorization form. You can also sign up for automatic bill pay by logging in to BAM.

In most cases, turning 26 means you will no longer be covered under your parent's health plan. However, turning 26 qualifies as a life event that makes you eligible for Special Enrollment. 

You can enroll in your employer's health plan or sign up for an individual health plan at that time. However, you will need to submit documentation before you can enroll. See the list in part 1b of the Special Enrollment Period: Valid Documents Chart para obtener más información.

If you have a self-insured group health plan with BCBSTX, your employer will send you a 1095-C form.

If you have a health plan directly from BCBSTX, you can get your 1095-B form one of three ways:

Print and fill out the 1095-B Tax Form Request and then mail it to:

When we receive this request, the form will be mailed to your address. The form will arrive within 30 days.

You can find digital versions of our forms using Buscador de formularios or the Forms & Documents link under the “My Account” tab on BAM.

Here's how to submit a form:

• Download the form you need and fill it out completely. 
• Log in to the BAM Message Center.  
• Create a new message and attach the completed form and any other supporting documents to it.
• Select a subject category from the drop-down menu. For example, if you are submitting a claim form, select "Claims Submission Attachments" from the drop-down menu.
  
• Click Send to send the message and submit the form.

You can submit a completed claim form one of two ways:

• Go to the BAM Message Center. Attach the completed claim form and the original bill from the provider to a new message. Then, select "Claims Submission Attachments" in the subject dropdown before sending the message.
• Mail the claim form and the original bill from your provider to the address listed on the form.
